It was a global phenomenon helping thousands of organisations implement something that became known as Behaviour Based Safety (BBS) \nIn 2010\, dr. Thomas Krause led a groundbreaking study on Serious Injuries and Fatalities (SIFs). This study demonstrated that only a small subset of all accidents have the potential to result in a SIF event. This principle is by now often referenced as the triangle within the triangle. \nAdditionally\, we have learned that the precursors to SIFs differ significantly from those associated with minor incidents. With this realisation\, how does this impact the BBS approach? What lessons could be learnt and is there now a different role that BBS should take? \nAt Krause Bell Group we know that leaders often fail to fully grasp how their behaviours and decisions influence not just systems\, but also the surrounding context and organisational culture. They may not recognise how these systems\, contexts\, and cultures shape employee behaviours. Furthermore\, if their BBS approach fails to provide them with sufficient feedback on SIF exposures a gap will be created with informing their future decision. \nHow can we fix this and move a BBS process to be more effective in strategically reducing risks? Additionally\, we have learned that the precursors to SIFs differ significantly from those associated with minor incidents. \nAt Krause Bell Group\, our work on SIF prevention starts with the fundamental premise: if we want to prevent fatalities\, we must reduce SIF exposure. Any effort we undertake—whether through training\, standards\, EHS systems\, or leadership development—will only be effective if it reduces or eliminates SIF exposure. However\, achieving this is easier said than done. Culture\, leadership behaviours and decisions have a profound impact. \nCulture – the way we do things around here – “eats strategy for breakfast\,” and often reduces the effectiveness of these initiatives significantly. While leaders often want to do the right thing\, they frequently lack the knowledge or tools to act effectively. \nLeaders often fail to fully grasp how their behaviors and decisions influence not just systems\, but also the surrounding context and organizational culture. They may not recognize how these systems\, contexts\, and cultures shape employee behaviors\, nor do they receive sufficient feedback on SIF exposures and the broader systems and culture to inform their future decision. \nHow can we fix this and implement a systematic\, effective process to strategically reduce risk? Leaders must first develop a thorough understanding of the key risks within their areas of responsibility.
