DESCRIPTION:Join Filip Coumans\, Thomas Meier and European colleagues for an online discussion on organizational decision making\, safety culture and fatality prevention.   \nSenior leaders frequently tell us of their concern for Serious Incidents and Fatalities (SIFs). They feel frustrated when investigations reveal they have a culture in which people don’t follow procedures designed to protect them.  It’s common for them to say\, “What was that person thinking?” and then ask\, “What should we be doing to create a culture that protects our people from serious and fatal events?”   \nMany leaders are unable to see the connection between decisions they make today and incidents that happen years from now\, causing harm to an operator in a plant or an engineer in the field.
