Their session focuses on how an analysis of 365+ decisions from actual events demonstrates how organizational choices—not worker error—interact over time to reveal a network of interdependent decisions that create serious exposure.

Traditional incident investigations over-index on proximate causes—worker actions, rule violations—and on management system gaps such as policies, procedures, training, and oversight.
This session introduces decision analysis, a structured approach that maps decision pathways and analyzes key choices to expose risks that remain invisible in traditional methods.
The presentation shares longitudinal findings from 365+ incidents, walks through case studies, uses real stories to highlight the insights, and introduces three intervention pathways to improve SIF prevention:
- Re-designing incident investigations to yield deeper systemic insight.
- Creating effective learning approaches to bring forward the voice of the worker.
- Embedding forward-looking practices so leaders can anticipate and act on systemic exposures.
Attendees will leave with examples of how hidden exposure can be surfaced and addressed. Decision analysis gives leaders—not just safety staff—a structured way to see how their choices, priorities, and trade-offs shape exposure.
