The 2010 study on Serious Injuries and Fatalities (SIFs), led by Dr. Thomas Krause, demonstrated that only a small subset of workplace incidents have the potential to result in a SIF event. This principle is now commonly known as the “triangle within the triangle.” We have also learned that the precursors to SIFs differ significantly from those associated with minor incidents.

At Krause Bell Group, our approach to SIF prevention begins with a fundamental premise: to prevent fatalities, organizations must reduce SIF exposure. Any initiative—whether focused on training, safety standards, EHS systems, or leadership development—will only be effective if it reduces or eliminates exposure to serious injury and fatality risks.

However, achieving this is easier said than done. Organizational culture, leadership behaviors, and business decisions all have a profound impact on workplace safety.

Culture—“the way we do things around here”—often “eats strategy for breakfast,” significantly reducing the effectiveness of safety initiatives. While leaders generally want to do the right thing, they frequently lack the knowledge, tools, or feedback needed to take effective action.

Leaders may not fully understand how their behaviors and decisions influence safety systems, the working environment, and organizational culture. They may also overlook how these factors shape employee behavior. Without sufficient feedback on SIF exposures, safety systems, and culture, leaders are unable to make fully informed decisions about serious incident prevention.

So how can organizations establish a systematic and effective process to reduce workplace risk? Leaders must first develop a thorough understanding of the most significant risks within their areas of responsibility. They must then recognize how their decisions affect those risks and take deliberate action to address them.
